New Patient Registration

appointment_1If you would like to register as a patient at The Riverside Practice, you should contact reception and one of the Receptionists will let you know what is required and give you the necessary forms to complete. 

Alternatively a Registration Form can be downloaded and printed onto paper from this website, for you to complete by hand and either bring into the practice yourself, Post or Fax to us. Please click on the tab "Registration Form" to access the link to this document.

All newly registered patients will be asked to complete a questionnaire and have a consultation with the Practice Nurse within one month of registering. This appointment enables the Practice Nurse to take note of your medical history, until we receive your medical records from your previous Doctor. Medical treatment is available from the date of initial registration.

To assist us with your care, please ensure that we have an up to date telephone number for you.

 

Practice Area

Our Practice area is Musselburgh, Wallyford, Whitecraig and part of Edinburgh. The area is bounded to the west by Brunstane Road as far as the roundabout at the Jewel on Milton Road, then following the Edinburgh A1 to Newcraighall Road, returning down Newcraighall Road to Musselburgh and including the village of Newcraighall. To the east, the area is bounded by the end of Ravensheugh Road, Wallyford Toll roundabout and to the south by the villages of Wallyford, Carberry and Whitecraig.

 

Moving Address Outwith the Practice Area

If you are a registered patient with the practice, and then move to an address outwith our practice area, you should register with a practice in the new area that you are living.

If you do not tell us that you have moved to an address outwith our practice area and remain registered, we will (if notified by any other Hospital or Agency that you have changed your home address to one outwith our practice area) write a letter to you - informing you that you will have to re-register with a practice in your new area. We will also send a letter to Primary Care for NHS Lothian, informing them that you are now living outwith our practice area, and that you should, therefore, be removed by them from our list of registered patients.
