From May 2023-April 2025, people with diabetes, stroke, chronic kidney disease, coronary heart disease, peripheral arterial disease and COPD will receive a reminder around the time of their birth month to book a ‘long-term conditions review’, and on an annual basis thereafter from the date you have your review. People with asthma or hypertension ONLY will be contacted about a review in due course.

Smear tests, blood pressure checks, contraception reviews and blood tests can also be pre-booked ahead of time at patient request. Please call for an appointment. Certain patients on the contraceptive pill and patch can fill in an online review form rather than calling to book an appointment – check the form to see if you’re eligible.
