Reviewing your asthma

Asthma Control Test – Adult

If you’re an adult and can do so, please fill in this online form. At the end, you’ll be given a score which tells you how well your asthma is currently controlled and the next steps to take.

If you’re a child or an adult and would prefer to have your asthma reviewed in person please call us during your birth month on 0300 790 6266 after 10am Monday-Friday, to book a review appointment with one of our specialist asthma nurses. Appointments can also be booked at the front desk.

If you ever have asthma symptoms that you feel need urgent assessment, please call us to book a same-day appointment, or call 111 or 999 (or go to A&E) as appropriate.

During the last 4 weeks, how much of the time has your asthma kept you from getting as much done at work, school or home?
During the last 4 weeks, how often have you had shortness of breath?
During the last 4 weeks, how often have your asthma symptoms (wheezing, coughing, shortness of breath, chest tightness or pain) woken you up at night or earlier than usual in the morning?
During the last 4 weeks, how often have you used your rescue inhaler or nebuliser medication?
How would you rate your asthma control during the last 4 weeks?
