We have two Community Linkworkers based at the Practice, who work for the charity Penumbra.
We all live with pressures in our daily life. Sometimes those pressures grow to the point that they affect our health and
wellbeing. There are lots of reasons for feeling under pressure – for example money worries, difficulty with housing or benefits, loneliness, living in a stressful relationship, experiencing bereavement, or perhaps struggling with parenting or caring responsibilities.
Your Community Linkworker can help you with these kind of day-to-day problems that affect your health. They’re here to listen to you, and hear what you think would help you to manage and reduce the pressures on you. They’ll help to link you with support and activities locally available that can help. This can be anything from specialist advice, to support to join in with health and wellbeing activities.
Appointments with Community Linkworkers
Any healthcare professional at the Practice can refer you to one of the Riverside Community Linkworkers, if you’re aged 18 or over. Please ask any GP, nurse or our specialist paramedic.
Once you’ve been referred, your Community Linkworker will get in touch within 1-2 weeks to offer you an appointment. They offer appointments at the Practice, via video or telephone call. Your first appointment is likely to take 30 minutes to an hour, with further sessions if needed.
Community Linkworkers are a really valuable part of our Practice team and we encourage patients to let us know, if you think you’d benefit from an appointment with them.
