It’s very useful for the Practice if you allow us to use your mobile number for text messaging. It saves the Practice (and the NHS) time and money.

If you allow us to use your mobile number, we’ll contact you by text with appointment reminders. We may also contact you about other important matters including inviting you for health reviews, to let you know that test results have been returned to us, or other important general Practice information. We won’t contact you about generic health promotion information that isn’t individual to you.

Please be aware that if you don’t consent to us contacting you by text (SMS) this will opt you out of ALL text messages from the Practice and you won’t receive appointment reminders, notification of test results and other important information about your health by text.

This form collects your name, date of birth, email, other personal information and medical details. This is to confirm you are registered with the practice, to allow the practice team to contact you and also to update your medical records held by the practice and our partners in the NHS. The internet is not a secure place, however, we have gone to great steps in making sure the information you submit to us is as secure as possible. We use SSL (Secure Socket Layer) certificates to encrypt the communication between your computer and our web server. If you are not completely happy to provide information via the internet please contact the practice directly.
