To register with us first please check your address falls within our Practice boundary area (see below) and then:
You can also collect hard copy forms at Reception, if you prefer.
It may take up to a week for us to register you after you send the forms in, so please wait until then before making an appointment. If you enter a mobile number on your registration form we’ll text you to let you know when your registration is complete. If you need urgent medical assistance before we process your registration, please call us on 0300 790 6266 or come to Reception to let us know – we’ll do our best to help.
**We can only set up repeat prescriptions for you once we get a list of these from your old Practice. If you want to speed this process up, you can request a copy of your medical summary from your old Practice and bring us a hard copy or email it to us. Please then use our online prescriptions forms to request that your repeat prescriptions are set up and add a note in the free text box to let us know that you’ve handed in your medical summary from your old Practice.**

Moving outside the Practice area?
If you’re registered with us and then move outside our Practice area, you’ll need to register with a GP Practice in the new area you’ve moved to. If you’ve moved outside our Practice area and remain registered with us, we’ll take steps to deregister you.
