This form is for repeat prescription and acute medication / special request medications only. For contraceptive pill orders, please call us to book an appointment with one of our Practice nurses.
Acute medications are medicines that you might have used before, but which have not been added to your repeat prescription requests – generally, this is because their use needs to be closely monitored by a doctor. Examples include many anti-depressants, medicines that can be addictive, or medicines that are subject to legal or clinical restrictions.
- If you tell us which local pharmacy you use we’ll send your prescription to them, so you can collect it there.
- If you request a mixture of repeat and acute medications we process the repeat ones first, so you get them as soon as possible. Prescriptions for acute medications will be sent afterwards.
- Please check your prescription for any notes sent by the GP. If they’ve asked you to book a medication review appointment with us please do it as soon as possible – don’t wait until your medication is about to run out.
Once you click submit, your prescription request will be sent to us. You won’t receive an email confirmation.
